Lacrosse Equipment Permission Form *- If any player wants to use a helmet, defense handle or goalie equipment for lacrosse camp, the parent must fill out & sign this permission form. The form must be returned to the coach or directly to me. Then it becomes the parents' responsibility to return the equipment to my house promptly after camp.
